Course Description

This course is designed to help clinicians understand the Social Worker’s role in End-of-Life Care and identify the Hospice and Palliative Care Program benefits. In this course, the Social Worker will identify the differences between the two programs, assess the needs of the patient, and make an appropriate referral based on condition and treatment preference.

Learner Outcomes

By the end of the workshop, participants will be able to:  

  • Identify the role of the social worker in End-of-Life Care; and 
  • Describe the use of Hospice and Palliative Care Programs.   

When participants return to the workplace, they will be able to:  

  • Assess which patients could benefit from Hospice or Palliative Care; and 
  • Make an appropriate referral to Hospice or Palliative Care.
