Course Description

This course teaches the basic steps to ensure a successful discharge from one healthcare setting to the next or to a patient’s home. During this seminar, we will look at what patients, families, and caregivers need to ensure a positive care transition, including a look at patient documentation, insurance, and in-home supports. We also will cover how to mediate with families and loved ones, as well as how to navigate potential ethical issues and conflicts with the care team.

Learner Outcomes

By the end of the workshop, participants will be able to:  

  • Recognize patient barriers to successful transition and learn how to mediate them; and 
  • Identify approaches to decrease readmissions.   

When participants return to the workplace, they will be able to:  

  • Create meaningful aftercare plans with the patient and their support network; 
  • Complete the necessary documentation and resources a patient must have for a stable discharge; and 
  • Work on an interdisciplinary team, especially when conflicts arise.
