Course Description

Record-keeping and documentation are equally important before, during, and after a clinical encounter. A common adage states that “If it isn’t written, it didn’t happen”.  Although documentation serves multiple essential purposes, it can be fraught with risk and liability.  This training will encompass clinical, administrative, and legal issues. Case studies will be examined to demonstrate ethical risks in the documentation. 

The aim of this training is to help practitioners approach documentation in a way that is not guided solely by mandated requirements but also in a way that is mutually beneficial to all stakeholders in the documentation:  the practitioner, the agency, the funding source, and most of all, the client. Participants will come away from the training with increased confidence and skill in providing clinical and ethical documentation that simultaneously benefits and protects the consumer, the agency, and the practitioner.

Learner Outcomes

At the end of this training participants will be able to: 

  1. Explain the need for high-quality documentation 

  1. Describe the components of good clinical documentation 

  1. Identify aspects of clinical documentation that present potential ethical problems 

  1. Distinguish between what is unethical and what is illegal in the context of documentation issues 

  1. Utilize a documentation decision tree

This training has been approved for 3 continuing education hours through co-sponsorship of Temple University Harrisburg, a PA State Board of Social Workers, Marriage and Family Therapists, and Professional Counselor pre-approved provider of continuing education. These credit hours satisfy requirements for LSW/LCSW/LMFT/LPC biennial license renewal. Certificates of attendance documenting continuing education hours will be issued by Temple University Harrisburg.
