CPHSSWCE0396 - Supporting Individuals and Families across the Lifespan Impacted by Illness: Navigating the Health Care System
Course Description
In the United States most individuals will have some interaction with the Health
Care system across their life span either for treatment of an acute or chronic illness. As Social Workers, we should have basic knowledge of those systems and the skills to effectively interact with health care providers to serve as advocates and assist with navigation. The multi-level impact of acute and chronic disease on the quality of life of individuals and families is significant. In this course we will discuss the history of Social Work and other Health professions in addressing illness and disease in different settings. We will also explore the variety of settings that Social Workers are employed in and best practices of working as a member of multidisciplinary teams.
We will introduce the Social Determinants of Health framework to contextualize the macro, mezzo and micro impacts of health and illness across the lifespan. We will further address working with individuals and families at the end of life.

Objectives:
At the end of this workshop, participants will be able to:
- Describe the biopsychosocial impact of chronic and acute illness when working with clients in multiple settings.
- Identify 2 key areas and issues for needs assessment for effective resource navigation and/or treatment planning.
- List at least 2 core competencies in advocating for clients' needs in Health Care Settings who have medical needs.
- Recognize the framework of the Social Determinants of Health and its impact on navigating individuals and families through the Health Care System.
