Temple is committed to the well-being of all members of our campus community. Each of us has an essential part to play in the effort to keep campus and the surrounding community safe.

On August 13, 2021, Philadelphia’s Board of Health announced new COVID-19 vaccination requirements for college campuses. All members of Temple’s community must be fully vaccinated by Oct. 15, 2021, unless they have a medical or religious exemption. Learn more about this new vaccination policy.

Students, faculty and staff who are fully vaccinated and who have uploaded their vaccine information to the health portal are not required to participate in the university’s surveillance testing program. Temple University will assume that you are unvaccinated until you provide evidence of being fully vaccinated.

The university’s mandatory surveillance testing program requires testing as frequently as twice weekly for those who are exempted from being vaccinated against COVID-19, and begins when you arrive on campus. Failure to comply with testing may result in disciplinary measures including but not limited to revocation of access to Temple’s buildings.

Instructions for completing this form:

  • Students who have not submitted COVID-19 vaccine information via the Non-Credit Covid-19 Compliance: Vaccination/Exemption/Remote-Only Documentation form are required to participate in testing. You must be tested at least once a week at a non-Temple provider (e.g. CVS, Walgreens, etc.).

  • PLEASE MAKE SURE YOUR NAME AND DATE OF BIRTH ARE ON ALL SUPPORTING DOCUMENTATION!
