Submit this form if you will be departing the U.S. and do not anticipate returning in the next five or more months. 

If this is your first time filling out this application,  click the "Apply Now" button in the lower right-hand corner.

If you are returning to complete your application, or to check on the status of your application click on  "Student Portal" in the upper left-hand corner. Scroll down to "My Applications" and you will be able to continue and submit your application

Please use this form to report your departure from the U.S. when you are departing permanently. Completing and returning this form to ISSS will enable us to update your SEVIS record correctly.

If you are unsure of your future plans, please come in during walk in hours to discuss your options with an international student advisor.
