Certificate of Eligibility (form I-20 or DS-2019)

You are seeking visa sponsorship from Temple University to obtain F1 or J1 student status

  • Degree Seeking Students or Students returning from a leave of absence that exceeded five months

  • Conditional Admission Program

  • Center for American Language and Culture

  • Beasley School of Law

  • Fox Specialized Masters Program


Immigration Reporting

Submit this form to report your arrival to Temple University.

SEVIS Updates (I-20/DS-2019)

Authorized Early Withdraw

You need to stop attending the university for a personal or academic reason


Exit Form

Complete if you will leave or have left Temple University and you are permanently departing the U.S. 


Extend your I-20 or DS-2019

Your I-20 is going to expire.


I-20 or DS-2019 Change of Academic Level

You have been admitted to a higher level of study (IELP => Degree-seeking) or you've completed one Master's degree and have been admitted to another Master's degree.

You have changed your visa status since being issued an I-20 or DS-2019.


Legal Break - Intensive English Language Program Students

You are in the IELP and have studied for 28 weeks in a row and want to take a break for one session.


Reactivate Your SEVIS record

You have been outside the U.S., for fewer than five months, after being granted an authorized early withdraw.


Reduced Course Load Request Form - Academic

You have an academic reason to be less than full time. Most commonly your first or last semester of study,


Reduced Course Load Request Form - Medical

You have a medical condition that requires you to be part-time.


Reinstatement Application


Transfer Out Request Form

You have been admitted to another university in the U.S. and will transfer your SEVIS record.

Optional Practical Training


Optional Practical Training Report Form for STEM OPT who need to report or update employment, change of address, etc.

As of March 23, 2018 students will report OPT employment through the SEVIS student portal. When your OPT is approved by USCIS you will receive an e-mail asking you to update your OPT through the portal. If your OPT is approved and you do not receive an email contact isss@temple.edu.

J1 Academic Training

You have been offered employment, directly related to your major field of study.


Immigration Reporting

Submit this application if your status has been changed from F1 or J1 to another status


STEM OPT Extension

Submit this application if you are on an authorized period of post-completion OPT, in a STEM field and your employer is enrolled in the e-verify program

All applications should be submitted to International Student and Scholar Services using the applications available below.

Be sure to use your AccessNet and AccessNet password. If this is your first time completing one of the ISSS applications, select the application and select "Apply Now".

If you are returning to complete an application you started earlier, or to check the status of your application:

  • Click on "Log In" on the upper right corner.
  • Log in with your AccessNet ID and AccessNet password.
  • Select "My Applications" from the menu on the left. Select the application you previously started and enter any missing information.
